Community Living Assistance and Support Services
(CLASS)

Community Living Assistance and Support Services (CLASS) provides home and community based services to people with disabilities or a related condition as a cost-effective alternative to placement in an intermediate care facility. A related condition is a disability with a primary condition other than mental retardation that affects the ability to function in daily life. CLASS is a Medicaid waiver program providing individualized services in the person’s home and is administered by the Department of Aging and Disability Services (DADS). UCP of North Texas provides Case Management Services for the CLASS program in the North Texas Area with regional offices in Dallas, Fort Worth, Lufkin and Longview. Available Services for CLASS (Back)

  • Case Management

  • Habilitation

  • Habilitation Training

  • Respite Care

  • Nursing Services

  • Physical Therapy

  • Speech Pathology

  • Occupational Therapy

  • Psychological Services

  • Adaptive Aides and Medical Supplies (up to $10,000/year)

  • Minor Home Modifications ($7,500/lifetime)

  • Consumer Directed Services (link to UCP CDS page)

  • Specialized Therapies (music therapy, therapeutic horseback riding, massage therapy, and recreational therapy)

 

Eligibility Requirements for CLASS (Back)

  • The onset of the disability must have originated before the age of 22.

  • Have a DADS approved initial individual service plan that does not exceed the program’s cost ceiling.

  • Have demonstrated a need for ongoing habilitation services and case management services.
